Bipolar Disorder Science News - National Institute of Mental Health Update

Five long years have passed and I have come to know bipolar disorder and the mania experience quite well. Though not my friend … it has not really been my enemy for a very long time.

Through the years, I have noticed more and more television commercials and print ads for medications of all types including mental disturbances. And as I review my own writings, I am reminded that we are fortunate to live in such historic times. Manic-depression in the 1900s’ meant that if you were stricken, you were institutionalized with murderers and the like. It was madness and mayhem as women were often abused by the very “caregivers” that were to care for them. Patients were starved, or killed, they were considered worse than thieves … they were the cast-offs of society. Science in those days included performing procedures like lobotomies on the mentally ill, it was another aspect of the horrific medical misjudgment and mismanagement of the day. Today researchers bring to consumers break-through science and as they discover the intricacies of the human brain they move revolutionarily forward. Knowing how the engine performs helps one obtain a greater appreciation for it. Today, patients are treated differently, medication helps greatly, and though we battle the stigma of title, patient … bipolar disorder (I,II,III) etc. we are ahead of the game. As researchers and scientists understand the clinical picture obtained from the information technology age of ours they will see clearly how the pieces come together to complete the puzzle. In this age of warp speed, we must only be ahead of the religious war to see our dreams come to fruition. Hollywood has made bipolar disorder a cutting edge illness. No longer the 1900s - it is 2008 and if bipolar disorder is the American buzz word than Hollywood has made the consumer an American icon.

There are many engines that pull the train of technology. One highly accredited resource is NAMI (National Institute of Mental Health). NAMI is the nation’s largest grassroots mental health organization dedicated to improving the lives of persons living with serious mental illness and their families. Founded in 1979, NAMI has become the nation’s voice on mental illness, a national organization including NAMI organizations in every state and in over 1,100 local communities across the country who join together to meet the NAMI mission through advocacy, research, support, and education. NAMI does need funding to continue their research. “Faster-Acting Medications for Bipolar Disorder’s Manic Phase May Be Feasible

New Research Pinpoints Potential Molecular Target in Brain Cells” NAMI, January 2008

Scientists may be able to develop faster-acting medications for the manic phase of bipolar disorder, new research shows. Current medications take several days to weeks to work, during which the extreme mood shifts of the disease may cause patients to engage in harmful behaviors, such as risky health behaviors or spending sprees. Bipolar disorder, also called manic-depressive illness, affects about 5.7 million Americans age 18 and older in any given year.

The faster medications would be aimed more directly at a molecular site on brain cells that current medications, such as lithium and valproate, reach through a slower, roundabout route. By targeting the site with a protein fragment they designed, NIMH scientists reduced manic-like behaviors and associated brain changes in rats. Jing Du, Ph.D., Husseini Manji, M.D., and colleagues published their results in the January 2 issue of The Journal of Neuroscience.

With further research, the molecular site could become a target for new medications for humans, or could point the way to other targets for new treatments, the scientists say. The site is an amino acid, serine 845 (S845), in the GluR1 subunit of the AMPA receptor. (See “About the Science.”).

The researchers also pinpointed a region of the brain that appears to be involved in mania: the CA1 region of the hippocampus, which feeds stored memories to the prefrontal cortex, the “active-thinking” part of the brain. In conclusion, this generation has come to know and benefit from more scientic discoveries than ever in the history of medicine. So it is with this knowledge that we must not forget our responsibility to contribute to further the cause. If in fact, 5.7+ million Americans’ will, at some time this year, experience a mental disorder, then it is our responsibility to contribute as we have done for the American Cancer Foundation and the American Heart Association and so many other fitting medical causes along the way. All very worthy causes, I beseech you to contribute as you can, to the National Alliance Mental Illness (NAMI.org). It is a fitting cause, it is a worthy contribution, and the science of the mind may someday be the difference between life or death for someone you love.

Is There a Bias in Media or is it Just an Urban Legend?

Have you ever wondered who ultimately has control of the media? This is a subject of concern to many. The question really isn’t so much about the ownership of a news organization as it is about the prevailing belief system of those who gather and report the news.

The primary impetus in journalism is to seek to answer a few key questions, for example questions like who, what when, where, why and how. This should satisfy the primary objective of journalism, however many assert this is only a starting point with a fixed agenda at the other end of the report.

In this debate the one thing both sides seem to be able to agree on is that the other side does not present news fairly.

Many contend that the bias of most media is liberal in worldview. This is attributed to radio, television and newsprint. The feeling is that news organizations most often side with liberal issues and will provide more news time to these issues than those who espouse a more conservative point of view. Some have even attempted to chronicle the instances of a liberal bias in the news and those reports seem to indicate that there may be either a liberal bias or at the very least a tendency to keep the conflicting points of view out of balance.

The lost of offenses seem long and unrelenting, but this issue is not one sided. If it were there would be no debate.

There are those who contend there are similar offenses on networks that purport to provide coverage that is more fairly balanced. In these cases those who espouse a more liberal worldview believe that the news coverage on these networks are tantamount to propaganda and misinformation. It is further suggested that these news organizations are the lapdog of the conservative right. As with our previous example there are those that will point out instances where coverage had a decidedly conservative bias with little coverage of any opposing point of view.

A growing contingent of online new distributors has taken the idea of news gathering and information sharing to a place of personal interest. Many individuals and smaller companies are providing news coverage online because they do not trust some of the better-known news gathering organizations. However, it could be argued that even these smaller organizations have a bias that is showing through.

I’d like to contend that there is always going to be a strong tendency to bias in news gathering and reporting. It is possible that a more liberal leaning newscaster will accentuate negative information about a conservative issue while the same would be true about a conservative reporter pointing out flaws in a story about a liberal issue.

It is the contention of some that this may not even be an issue of ill intent. The negative jabs at the opposing perspective may come from an unconscious bias that is not consciously recognized by the writer or reporter. However, this scenario is often recognized quickly by those who have a different perspective on the issue.

Is there a bias in media reporting? If so does that bias lean on one direction more than another? If so, who is responsible?

Anatomy of the Skin - The Basis for Understanding Aging Lines and Wrinkles

The epidermis is the most superficial layer of the skin and provides the first barrier or protection from the invasion of foreign substances into the body. The principle cell of the epidermis is called a keratinocyte. The epidermis is subdivided into five layers or strata, the stratum germinativum (SG), the stratum spinosum (SS), the stratum granulosum (SGR), the stratum lucidum (not seen in this photomicrograph) and the stratum corneum (SC) in which a keratinocye gradually migates to the surface and is sloughed off in a process called desquamation.

The condition of the epidermis determines how “fresh” your skin looks and also how well your skin absorbs and holds moisture. Spots, pores, evenness, porphyrins, and UV spots all manifest themselves in our epidermis.

Wrinkles, however, are formed from changes in the dermis.

The dermis assumes the important functions of thermoregulation and supports the vascular network to supply the avascular epidermis with nutrients. The dermis is typically subdivided into two zones, a papillary layer and a reticular layer. The dermis contains mostly fibroblasts which are responsible for secreting collagen, elastin and ground substance that give the support and elasticity of the skin. Also present are immune cells that are involved in defense against foreign invaders passing through the epidermis.

The papillary dermis (PD) contains vascular networks that have two important functions. The first being to support the avascular epidermis with vital nutrients and secondly to provide a network for thermoregulation. The vasculature is organized so that by increasing or decreasing blood flow, heat can either be conserved or dissipated. The vasculature interdigitates in areas called dermal papillae (DP). The papillary dermis also contains the free sensory nerve endings and structures called Meissner’s corpuscles in highly sensitive areas.

The reticular layer of the dermis (RD) consists of dense irregular connective tissue, this differs from the papillary layer, which is made up of mainly loose connective tissue (note the difference in the number of cells). The reticular layer of the dermis is important in giving the skin its overall strength and elasticity, as well as housing other important epithelial derived structures such as glands and hair follicles. This reticular layer is our target for collagen production.

Collagen

Collagen is a member of the connective tissues: cartilage, bone, tendons, fascia, ligaments, and blood vessels. The character of any connective tissue is determined by the function of the specific cells that compromise that tissue. The major fibrous proteins are collagen and elastin. The predominant nonfibrous components are the complex carbohydrates mainly: hyaluronic acid, the proteoglycans, and glycoproteins. Collagens are a class of proteins, members of which have chemical and structural features in common, but each is a product of a specific gene (hence, the specific connective tissue disorders). One property of all collagen molecules is the unique triple helix, a particular conformation of the three component polypeptide (alpha) chains, each containing approximately 1000 amino acids. The conformation of the chains is determined by the amino acid content, with glycine constituting a third of the total and occurring at every third position in the amino acid sequence. There are two major classes of collagen: interstitial and pericellular. Interstitial collagens are the major collagen of the skin and essentially exclusive type of bone (type I); articular cartilage and nucleus pulposus (type II); and the collagen present in the skin, blood vessel walls, and the matrix of parenchymal organs (type III). Pericellular collagens are types of IV and V and predominate in basement membranes.

Biosynthesis

Biosynthesis of the collagen chains is a multistep process in which a precursor form (procollagen) is first synthesized, with peptide extensions at either end. During synthesis, several amino acids are uniquely modified post-translationally (after incorporation into the polypeptide chains). These post-translational modifications include hydrozylation of praline residues (hydroxyproline) and lysine residues (hydroxylysine) and addition of sugars (glucose and galactose) to the hydroxylysines, and formation of the hydroxylysing and lysine aldehydes. Specific proteases act to cleave off the extensions of the procollagens to produce the processed collagen molecules, which can then polymerize to form fibrils and fibers. Our lasers target the cell mediators which stimulate the production of procollagen I & III.

Aging

Cutaneous (skin) aging is a complex biological phenomenon. Disorders in aged skin are more the result of photoaging (UVR) than from chronological aging. There is gradual atrophy of the dermis and epidermis and a massive accumulation of abnormal elastic tissue with its associated microfibrils and proteoglycans. These elements replace the normal collagen rich dermal foundation, leading to a weakened and thinner dermal layer. UVR consists of UVA (320-400nm) + UVB (280-320nm) and UVC which at the present time does not penetrate our atmosphere. UVA light is the prime offending agent for collagen loss and subsequent wrinkles. UVB’s shorter wavelength only affects the epidermis. As the dermal layer “sags” so then follows the epidermis, and wrinkles appear. This decrease in collagen results more from increased deterioration (UVA) rather than from a significant decrease in production (age). The term solar elastosis is used to describe photodamaged skin.

The thickness of the skin changes with age: young skin gradually gets thicker until the age of about 20 years, after which there is a gradual atrophy of the dermis. At the level of the papillary dermis, a relatively nonechogenic band appears and develops with age. The exact reasons for the appearance of this shadow are unknown, but it corresponds to a homogenization of the papillary dermis, the local disappearance of elastic and collagen fibers, and their replacement with an undifferentiated matrix of hydrated gycosaminoglycans (elastotic tissue). The extent of this shadow steadily increases with age and is a reliable marker for skin aging (loss of collagen density).

This shadow is found in both types of aging skin: photo and chronological. An increase in dermal thickness from laser therapy is found in the upper dermis and is due to an increase in the collagen fibers and not the other components of the matrix.

The laser heat causes a partial denaturation of the older collagen and accelerates the new collagen synthesis process by the fibroblasts and the deposition of new glycosominoglycans and leads to a reabsorption of elastotic material. An increase in the echogenic band in the upper dermis is easily visualized with ultrasound and is directly attributable to an increase in collagen. Two possible benefits occur with laser collagen remodeling therapy. The thickness of the dermis increases and/or the density of the collagen increases.

Anti-Aging

Anti-aging modalities are classified by type:

Type I therapy is directed at the epidermis (microdermabrasion/chemical peels/laser)

Type II therapy is directed at the dermis (laser/chemical peels/ultrasound)

Type III therapy is surgical or injectable (face Lift, botox, fillers).

Obama says offshore drilling stance nothing new

Sen. Barack Obama responded Saturday to criticism that he has changed his position on opposing offshore oil drilling.

Change management in organizations is about to be faced by a new critical challenge.
Over and above the fact that on average 18% of staff are actually actively working against the interests of the company, and another 61% don’t really care (1), the current escalation in financial and social pressures are making change programs twice as dangerous to proceed with.

If you are senior management and have done your homework you will already know that 70% of organizational change or cultural change programs fail to meet the requirements of the project targets. Most just fail completely and nothing changes at all. Yet we still pursue change without understanding the basic core necessities of successful change.

Unless you can create congruency between your goals and the goals of the people and management within your organisation, you will be fighting an uphill battle. If you are a student of Sun Tzu’s Art of War you will know that one never enters or even considers such a dangerous position. With the fear that is endemic in the current global economic meltdown and the extra pressures that this places on individuals, the added high internal stress levels for both organizations and its individuals are making any change program very difficult.

So here is what I suggest. Throw out your current plans and review two things.

First review if the change in the way the company operates is really necessary to survive or grow, or whether the project can be delayed for 12 months. If it is necessary, be prepared to spend the extra money to ensure the fight is on your terms.

Second, if you need to proceed don’t employ any Change Manager or HR staff that are not fully aware of effective methods of stress management and cultural change within organizations. As part of this awareness you also need to ensure that your management team is up to the job as they will be the direct interface with all staff during the project roll out. I strongly suggest that part of the plan be to incorporate a specialist in social engineering with a strong background in stress and trauma counseling to interface with the Project and Change Managers.

Do these things and you will potentially save hundreds of thousands or millions of dollars, and maybe save your company from extinction.

I know it all sounds so dramatic, but in these times of economic and social unrest you are sailing on dangerous waters. Don’t set sail with out the necessary safety equipment on board.
